The ADV/Dis-ADV thing is pretty easy to fix, you just need to adjust the shortcut keys for midi. Go to the settings tab then Configure Controls. Look for midi and change the shortcuts.
To revert to the dnd5e 2.4.1 compatible version uninstall midi-qol and reinstall it. You must make a bakeup of your 2.4.1. world before moving to dnd5e 3.0+ to be able to go back to dnd5e 2.4.1; Midi-qol provides combat automation with almost all features being optional. You can put an active effect on the character that reminds you to roll saves vs poison with advantage. It works with Midi but it really only works if you're not fast-forwarding rolls since it's adding a little message to the dialog. If you're fast-forwarding rolls then that dialog is skipped. The module has loads of feats, actions, spells, items and equipment premade which makes it much easier to get going without having to fiddle wtih sheets, stats and rolls. When I use enemies like Swarms that are resistant to all physical damage, it works fine. In fact, all resistances other than this "non magical" variety work as expected, yet regardless of the option I choose in midi's workflow settings or import (from SRD) and fiddle with items. It's odd, to say the least. Check your monster.
Take a look on the Midi-QoL settings, there's a configuration to automatically transfer effects on a failed save, that may not be selected. Try using some Midi-QoL/DAE SRD sample Item, that apply an effect, like bless, and see if they work. If they work the problem is in the items you made, try using the sample items as a base to make yours.Test it on a fire resistant monster like the Specter. You make need the modules Automated Animations and Midi-QOL. I highly suggest Midi-QOL to make your life as a DM much better becuase it can handle damage, To Hit checks, and saving throws automatically.
