I managed to wrap up my goal to enter all Ascendant Challenges from out of map by finally adding Keep of Honed Edges to the list. The trick was to get their via the Confluence, which as u/JewBoy300 reminded me, does not have a box once you load the adjoining regions. Unfortunately, all of those are no sparrow zones, so you can't just go wherever you want.

What you want to do is shoot, then while continuing to hold the ADS button also hold the reload button. This reloads a honed edge shot instantly - there's only a single reload. Holding the ADS button is not technically required but the timing is much more generous.
